Student Directory Form

Each year, the Susan Lindgren PTSO creates and provides to each student a directory that contains student, parent and staff contact information. It is frequently used by parents to set up play dates and carpools, address birthday invites, and contact school staff.

Please indicate how you would like your contact information to appear in the directory by submitting this form by the last Friday in September. You only need to submit one form per family. If you do not submit this form, only your student’s name and address (as listed in the district’s records) will be printed in the directory.

If you do not wish to have any information about your student printed in the directory, please stop by the Susan Lindgren office and complete a “Denial of Release of Directory Information Form” by the last Friday in September.
